Noninvasive mechanical Ventilation (NIV) 非侵入性呼吸器
Noninvasive positive pressure ventilators, NIPPV
(1) Strong recommendation
- COPD AE with respiratory failure (要合併酸)
- Cardiogenic pulmonary edema with respiratory failure (要合併酸)
(2) Conditional recommendation
- 免疫不全, 術後, 胸腔創傷 所導致的呼吸衰竭
- 接受緩和醫療的患者
- 預防拔管後的呼吸衰竭 (Post-extubation in high-risk patients)
- 促進高二氧化碳性呼吸衰竭患者脫離侵入性通氣設備
(3) Conditional recommendation against
- 不建議用來預防 COPD AE 患者出現呼吸衰竭 (Prevention of hypercapnea in COPD AE)
- 不建議用於拔管後呼吸衰竭的患者 (Post-extubation respiratory failure)
- 對 De novo respiratory failure 沒有建議
- 使用 NIPPV 期間, Tidal volume > 9 ml/kg 是失敗的預測因子, 應儘早插管
- Severe ARDS (P/F < 150 mmHg) 的病人使用 NIPPV 失敗率很高, 建議直接插管
- High-flow nasal cannula 在 Acute hypoxemia respiratory failure 似乎有好處, 其他情況則尚在試驗中
COPD AE 約 20% 的患者出現 Hypercapnic respiratory failure
當無法達到充足的肺泡通氣, 開始出現淺短的呼吸型態 (RR↑, Tidal volume↓)
最後導致血液中 CO2 retention
COPD AE, 若尚未達到酸中毒的程度, 不建議用NIV來預防呼吸性酸中毒
COPD AE, 若進展至呼吸衰竭 ( COPD AE + pH<7.35) 建議使用 BiPAP
COPD AE 考慮插管的病人, 可考慮先用BiPAP, 除非患者的狀況急性惡化
HF with pul edema 進而造成呼吸衰竭
機轉包括肺泡積水, 呼吸系統的順應性下降
NIV 可以改善呼吸, 減少左心室的 afterload
Cardiogenic pul edema 的呼吸衰竭, 建議使用 NIV (包括 CPAP, BiPAP)
急性氣喘, 流行性病毒所造成的呼吸衰竭, 目前證據力不足
術後的患者, 可能會因為橫膈膜功能不良, 導致呼吸衰竭
( 影響到橫膈膜的手術, 麻醉, 術後疼痛 )
在術後產生呼吸衰竭的患者, 建議使用 NIV
拔管後有高風險產生呼吸衰竭的患者
建議使用NIV來預防呼吸衰竭的產生
但非高風險者, 並不建議使用NIV來預防

BiPAP Can help with Type 1 (Hypoxemic) and Type 2 (Hypercarbic) respiratory failure
Two Settings on BPAP :
ePAP : Range 0 - 20cmH20.
Remember this functions like CPAP, so if you have a patient with pure COPD exacerbation and no Hypoxemia,
you could start this at 0 cmH20
Remember it’s the delta or difference in the iPAP and ePAP that affects TV.
So don’t just go up on both settings in hypercarbic respiratory failure
