TIA symptoms typically last < 1 - 2 hours
Duration of symptoms is an unreliable discriminator between TIA and infarction

Large vessel occlusions (Crtical stroke) are the most likely to benefit from thrombectomy 
ICA, MCA, and some ACA occlusions

- Uncommon but important cause of young stroke (10% to 25% of cases)
  在年輕型腦中風占了很重要的角色
- Especially in young and middle-aged patients
- Prominent risk factor : history of neck trauma in the days to weeks prior to presentation
  除了外傷也可能是運動傷害, 頸部按摩 
 
  Other risk factor : 
  Connective tissue disease, history of migraine, large vessel arteriopathies and hypertension.
  據統計有 1/4 的頸動脈剝離的患者以前曾有偏頭痛的病史
  很多論文提到病人會抱怨這次頭痛與之前不同
  此類病患與偏頭痛的病患最大的差異, 就是脖子會有頸部血管按壓的疼痛（Carotidynia）
  甚至有病人的脖子會痛到無法讓醫師碰觸或轉動脖子
- 先有 Unilateral headache (68%), neck pain (39%), or face pain (10%)
  Symptoms may be transient or persistent
  以上 Preceding symptoms 再發展出 neurologic symptoms 可能是 hours to 14 days
  典型的發作模式是先疼痛數日後發生中風, 期間可能伴隨局部神經學症狀
 

69 y/o male
DM + HTN for more than 10 years without control
chief complaint : left side weakness
NE findings :
Left central type facial palsy
Left side weakness - muscle power 上肢 5/2 , 下肢 5/4
Left babinski +
Left lower limb vibration 較差
* Localization :
有手有腳
level 至少是在 C spine 的 spinal cord 以上
Lesion 應該在 right side
vibration 走 posterior column 在 medulla 高位交叉
配合 lesion 在右邊, 所以定位 level 提高到 medulla 交叉以上
CN7 表現的是 Central type palsy
代表影響到的位置是在 CN7 nucleus 以上整條 Corticobulbar tract 都可以
CN7 nucleus 在 lower pons 
定位範圍大概就是從 Brainstem 的 lower pons 一直到 cortex  (right side)
病人沒有什麼 cortical function 問題
沒有 aphasia, anopsia, Agnosia, apraxia, hemi neglect
conscious 也很好
所以至少不像是大片的 cortex infarction
更往 lacunar infarction 想 ( Mixed sensorimotor type  ? )
最後位置是 right medial pons infarction ( 上 ~ 中 )
 

 

Age 18 - 50 years
More favorable outcome -> Treat these patients aggressively.
