    Myopathy 和 LMN 差別在 Fasciculation  !!



 

* Cortical spinal ( Pyramidal tract ) 
   自主隨意運動
 

• Lateral 58% 遠端肌肉隨意 :
  Primary moter cortex
  Internal capsule
  Midbrain 腹側大腦腳
  Pontine base
  Medulla pyramidal decussation 錐體交叉到對側
  對側側索一路到 Ant horn cell 
  之後就是 LMN
  
   
• Anterior 15% 近端肌肉隨意 :
  Medulla 不交叉
  同側前索才交叉
  到對側 Ant horn cell 
  之後就是 LMN

* Corticalbulbar :

和 Cortical spinal 基本上都一起 ( 路過大腦腳 )
Corticalbulbar 走到自己神經核的位置就變成 LMN 出去

基本上都是 bilateral innervation !!!! 

只有 下半臉的 CN7 +12 才是 單由對側 控制 !!!! 

* Vibration, Propriception tract, light touch :
      ( Posterior column )

      Peripheral n 
     背根神經節 dorsal root ganglion ( 1. )
     同側後索 ( 薄束 -下肢 / 楔形束 -上肢 )
     medulla 薄核楔形核  ( 2. )
     medulla 高位交叉至對側 medial lemniscus 內側蹄系
     thalamus  ( 3. )
     cortex 
     

* 溫痛 : 

  Peripheral n 
  背根神經節 dorsal root ganglion  ( 1. )
  dorsal horn cell 的 substantia gelatinosa 後角細胞 ( 2. )
  低位交叉至對側
  對側 lateral spinothalamic tract 
  thalamus  ( 3. )
  cortex 

* Hemi cord : 
 

• 同側肢體 motor
• 同側 震動本體感覺
• 對側 溫痛
• 同側 level 的 LMN 也會被壓到 Ant horn cell 

1. Transverse cord : 截面以下全面喪失
2. Hemicord : 同側本體震動,  同側運動, 對側溫痛
    
3. Central cord 小 : 只影響 lateral spinalthalamic 溫痛
                              從內側的 C spine 開始影響
4. Central cord 大 :  也幾乎是全部 ~
    
5. Posterior cord : 以下雙側本體震動
6. Anterior cord : 以下全面運動, 溫痛
