IICP is associated with certain characteristic ECG changes :
- Widespread giant T inversions = Cerebral T waves
- QT prolongation
- Bradycardia (Cushing reflex – indicates imminent brainstem herniation
- Massive ICH
- Severe IICP
- Grave prognosis
CNS catastrophe, especially hemorrhagic stroke
They are unusual in a small ischemic stroke.

Other possible ECG changes that may be seen :
- STE / STD - may mimic myocardial ischaemia or pericarditis.
- Increased U wave amplitude.
- Other rhythm disturbances :
sinus tachycardia, junctional rhythms, PVC, Af
