RLQ D/D: 

1. Appen
2. Diverticulitis
3. TOA, PID
4. Ovarian cyst diz ( torsion, rupture )
5. Ureter stone diz
6. Cron's 
7. Colon ca
    
8. Mesenteric adenitis- ileitis complex
9. Primary epiploic appendagitis ( PEA ) 腸脂垂發炎
10. Miscellaneous : typhlitis, colitis, ileocecal junction
                            盲腸炎 好發 chemo Neutropenia 病人

RLQ pain :

1. Appen : 40%
2. GI : 30 %
3. GYN : 15 %
4. Others 
    

Normal appen : 

1. Outer Diameter 通常 < 6 mm
   但即使 > 1 cm 仍能是 normal 
   Diameter 非絕對標準
2. Wall 正常是 1~2 mm
3. Appen 內都是 air 絕對正常
4. Contrast 能流進去的 也絕對正常
    

Simple appen :  記得 trace 到最後喔 !

1. Dilated outer diameter > 8 mm
    
2. Wall thickening > 3 mm
   ( 和其他 bowel wall 比較 )
    
3. Wall hyper enhaced 代表有發炎 ( 正常是不顯影的 )
   和 psoas muscle 相比 更白就代表 hyper enhaced
   或和其他 bowel 比較
    
4. 周圍有 fat stranding !!!!!!!!!!!!!!
    
5. Contrast 流不進去的
6. 有 fecalith 在裡面的 ( RLQ pain + KUB RLQ 找到疑似 stone 就幾乎可以確診 appen )
   
   
   Simple appen 就去開刀 appendectomy !!
    

Complicated appen  ( Ruptured ): 

形成 Complicated localized inflammatory mass ( abcess )
則不開刀, anti 6-8 w 再考慮開刀

若破了但沒有形成 abcess 還是可以開刀 

Complicated appen  ( Rupture ) 是 simple 再加上 

1. Wall defect
    
2. Abcess, phlegmon 爛泥巴
   
   appen 周圍不只是 exudate, fat stranding
   還出現不均勻的enhaced 的 soft tissue 
    
3. Extraluminal appendicolith 石頭跑到外面啦 !
4. Extraluminal free air
    
5. Peritoneal hyper enhanced
