定義 :

 Infection, Ulceration, Deep tissue destruction + Neurologic & Peripheral arterial disease

三大成因 :

1.  Peripheral neuropathy  導致對痛覺和溫度無感, 易受傷, self care 不佳
                                                     導致 small muscle wasting,
                                                     足弓變形 壓力不平均容易 callus formation 最後變 ulcer
    
2. Autonomic dysfunction  導致 blood flow 改變, foot vein distension 
                                                      不容易 sweating 導致 dry skin
    
3. Peripheral artery disease  導致 blood flow 差, 容易 infection, wound healing 差

基本上是 Neuropathy + Infection + Ischemia 的鐵三角 !!

Wet gangere 合併 infection 裡面有 pus
會很臭 odor 
需要 anti + debridement
DM foot 的 infection 通常都 mix flora 
更嚴重的感染甚至會 Osteomylitis

Dry gangere 可能可以自己掉 autoamputation

 

Treatment : 

首先目標是 Wound bed prepare
 

• Dressing
• Debride
• Negative pressure therapy 負壓幫忙增加血流 吸走 pus
• Hyperbaric oxygen 高壓氧

再來等他長 or STSG, FTSG... 來做 Reconstruction
