呼吸抑制解藥：Naloxone
* 止痛藥效果 :
Non - opioids
Weak - opioid :
Tramal 有 Ceiling effect
使用至相當劑量後, 即使劑量再增加其止痛效果也不增加
剩副作用增加
劑量大概是口服 morphine 的 1/5
Ultracet, Tramol 有暈吐等副作用, 可以避免成癮
Strong - opioids : 盡量不選, 會上癮
Chronic pain 的病人可考慮加 TCA
口服 (PO) 與注射 (IV) 劑型嗎啡的轉換為 3:1
PO 30 mg = IV 10 mg
IV 10 mg Morphone = 100 mcg IV Fentanyl
1 amp Fentanyl : 0.5 mg (500 mcg)
Fentanyl patch
先將每日嗎啡的劑量加總
根據總劑量再與持續釋放型貼片轉換
90~134 mg/day = 25 mcg/hr *1
135~224 mg/day = 25 mcg/hr *2 or 50 mcg/hr *1
25 mcg/hr patch 約等於 20 mg IV morphine (60 mg PO)
Fantanyl patch 止痛強度約是 morphine 的 100 倍
一般可以 持續 72 小時 (Q3D使用)
首次使用的患者
需 12~14 hr 才能達到有效止痛
使用第一片貼片後的初始 18 hr
可能需要適時重疊給予短效 morphine
而在貼片撤除後, 藥效仍能維持 16~24 hr
只用於慢性疼痛
不可第一次直接使用貼片, 有呼吸抑制風險
Tramadol
30 mg Morphine => 120 mg Tramadol (1:4)
300 mg Tramadol => 30 mg Morphine (10:1)
為了避免Tramadol 轉換為嗎啡時的劑量過高
一般建議 150 mg 換 30 mg (5:1)
但也有更保守建議到 10:1

Oxycodone
有相等止痛劑量, 但無轉換劑量建議
消化道症狀較少
教科書中有嗎啡與 Oxycodone 的相等止痛劑量 (equianalgesic dose)
10 mg IV Morphine 換 20 mg Oxycodone
根據 Oxycodone PR (持續釋放劑型) 仿單建議
目前並沒有臨床試驗佐證的轉換劑量建議
因此建議以 10 mg Q12H 起始劑量 + OxyNorm Q6H prn
開始調整至適當的止痛效果
OxyNorm 口服短效 Q6H prn 使用
OxyContin 口服長效
但 onset 約 10-15分
急性疼痛 :
已給止痛, 病人急性疼痛 STAT 劑量
- 1/6 Daily dose Q4H prn
- 如果 四小時內需給兩次 或 一天給三次以上 : 考慮原本每日止痛加量
藥品轉換時需考慮劑量增減
如果病人已耐受且疼痛受到控制
則鴉片類藥品轉換時, 會降低 25% 的劑量
但若病人過去使用的鴉片類止痛藥品並沒有達到止痛目標
則一般建議以 100% 轉換後劑量再加上 25% 劑量做為轉換後的起始劑量
Opioid 常見副作用久了都會 tolerance
只有針狀瞳孔 & 便秘 不會消失
記得開軟便藥
Fresh user : 噁心嘔吐約 3-7天 tolerate, 給 Novamine, Primperam
Initial IV opioid doses in the elderly are typically half those used in younger adults
e.g., morphine 0.05 milligram/kg
Adverse effects of opioids :
Nausea, Vomiting, Constipation, Pruritus, Urinary retention, Sedation, and Respiratory depression
Concern of Opioid use :
Respiratory depression, Facilitating drug-seeking behavior
Altered mental status, Hemodynamic instability, or multisystem trauma
Renal excretion is a major route of elimination for such pharmacologically active opioid metabolites.
In patients with renal failure, hydromorphone and fentanyl are the preferred opioids.
Morphine IV
Peak of action : 15 - 20 minutes
Half-life : 1.5 - 2 hours in healthy young adults
Duration : 3 - 4 hours.
An appropriate loading dose for acute severe pain is 0.1 - 0.15 mg/kg IV of ideal body weight,
augmented by repeated doses of approximately half the initial dose every 5 -15 minutes,
depending on the severity of the pain and patient response.
WHO 止痛指引
1. 能嘴巴吃就不打針 by oral
2. 定時 by clock
3. 階梯式 by ladder
- 輕度疼痛 : 可用 Acetaminophen, NSAIDs
- 中度疼痛 : 可用 Codeine, Tramadol, Ultracet, 或是強效但低劑量使用
- 強烈疼痛 : 可用 Morphine, Fentanyl, Oxycodone
此外有輔助藥品如 : BZD, TCA, 某些抗癲癇藥品或類固醇
Morphine pump :
10mg * 2 amp in NS 500 ml run 12.5 ml/hr (0.5 mg/hr)
3 - 4 小時可上調一次
run 25 ml/hr (1 mg/hr), run 37.5 ml/hr (1.5 mg/hr),
一天可上調四次
