* GNR 
  Facultative intracellular pathogen

 

 

* 和 E. coli, K.p, Proteus 都屬腸內菌科
  Salmonella 是屬名

• Family : Enterobacteriaceae
• Genus : Salmonella
• Species : S. bongori  &  S. enterica

 

* S. enterica 可以再藉由不同 antigen 分成 2500 種 serotypes 血清型

• O antigen : LPS
• H antigen : Flagellin
• Vi antigen : Capsule

* O antigens可再分成  :  A、B、C1、C2、D、E  groups 

 !! 臨床上重要的是區分 Typhoid or Non typhoid !! 

Typhoid 就是 傷寒熱
Non typhoid 一般只會 AGE 
非常非常少數才會像Typhoid 變成 bacteremia 
 

 

一般驗出 Salmonella 若是 Typhi 
會特別寫出是 S. typhi 傷寒
單純寫 Salmonella group D 代表是 Non typhoid 
( 寫出 Group ABCD 就代表是 Non typhi )

Typhoid fever 傷寒熱 

* 主要致病菌就是 S. typhi   &  S. paratyphi 

* Human only 所以只會人傳人

* Transmission : 
    是藉由被帶原者糞便汙染的食物和水
    Contaminated food, water
    Travel related  
    所以傷寒+副傷寒也是 第二級法定傳染病

 

* Risk group : 

1. Reduced gastric acid ( Antacid ) 胃酸和殺菌功能有關
2. Bowel mucosa injury ( IBD, Antibiotic used, post GI surgery )
3. HIV
4. Malnutrition
5. Splenectomy, Sickle cell disease

 

* Incubation period : 3 – 21 days 

 

一開始去淋巴血液
之後再去肝膽骨髓脾臟
 

• Blood culture : 
  Week 1  :  90 % (+) 
  Week 3  :  50 % (+) 
   
• Stool culture : 
  Week 1  :  usually (-)
  Week 3  :  90% (+) if untreated

 

* Symptoms : 

• High Fever, Chillness
• Headache
• Fatigue, Anorexia
• Abdominal pain
• Diarrhea, constipation

 

* Signs :

• Rose spots
• Relative bradycardia
• Hepatosplenomegaly

 

 

* GI Complication : 

• PPU
• GI bleeding

* Other Complication : 

• Meningitis
• Hepatitis
• Liver, spleen abcess
• Cholecystitis
• Myocarditis
• IE
• Pneumonia
• Orchitis

* Asymptomatic carrier :  
   5%, GB, biliary tract Stool culture (+)

Non typhoid : 

* S. typhimurium &  S. enteritidis
 

* Zoonotic 人畜共通
 

* Transmission :
   Contaminated food ( Egg, Meat, Poultry, Seafood) 
   這位永生難忘的 case 有接觸海鮮
   也記得 SLE 女病人驗出 Stool salmonella 
   老師一直交代不能吃雞蛋生冷
   Reptiles pets 國外很多案例是養了爬蟲類而感染
 

* High risk group : similar with typhoid 

 

 

一般 Non typhoid 感染
只會造成 AGE 
12 - 72 hr 後
N/V, fever, diarrhea, abd pain cramping pain

另外 salmonella 的 AGE 是屬於 Inflammatory diarrhea
所依 Stool OB, WBC 會 +

 

* Enteric fever / Bacteremia : 

只有在 5 % 的 non typhoid infection 會造成 Bacteremia
症狀很像 typhoid fever
但更嚴重可能致命
No rose spots & relative bradycardia

 

* Risk group :

1. Children ( 佔所有 non typhoid bacteremia 的 65 % )
2. Elderly
3. Immunocompromised !!!!!! 

           HIV !!
           Corticosteroid use
           Autoimmune disease ( SLE )    
           Cancer
           DM    
           Cirrosis    
           ESRD    
           IBD
           Hemoglobinopathies  ( Sickle cell anemia )
           Transplantation
           Suppression of gastric acid
           Malnutrition
           Recent antibiotic use

 

* 其中又有 10 % - 30 % 可能  develop focal infection : 

• Meningitis
• Pneumonia
• UTI
• Infectious endarteritis
• mycotic aneurysm (Endovascular infection)
• Septic arthritis, Osteomyelitis
• GB, liver, spleen abcess 
• Cholangitis

 

* 所以我的病人做了 : 
- Cardiac echo
- whole body CT
- Gallium scan
