* 三層 Hyperechoic : Skin, Fascia, Bone
  Skin 和 Fascia 之間是 subcutaneos fat 
  Fasia 到 bone 之間以 muscle 為主

 

* Linear probe ( 5-10 MHz )

1. 創造出好的 window : 大量 jelly, IV bag, 手套裝水
2. 兩邊比較
3. transverse + longitudinal

Cobble stone pattern : 

Celluits - subcutaneos fat 發炎, echogenicity 上升變亮
                   有 edema 
 

 

兩層紅線中的 subcutaneous space 變寬, cobble stone pattern, 有 edema 的一些 fluid
 

* 第二個重點 : 可以看出 abcess, wall 內有 低回音 fluid accumulation
   也可以用 probe 壓壓看 ( Echo squeeze sign ), 可以被擠則不像 mass like, 像是 abcess

 

Joint fluid aspiration : Septic arthritis, Knee joint....

Suprapatellar bursa 和 joint capsule 有相連

Foreign body : 

foreign body 放久了
旁邊會有 fluid accumulation
很難找 要細心比較兩側 ~

Bone facture : 

Cortical line 是否 smooth 沒有裂痕

 

Tendon tear : 

紅點範圍是 Achilles tendon, 呈現 hyperechoic 絲束狀
藍點是 Calcaneus bone

下圖可看到  Achilles tendon tear 分開 !!!
更可以 passive 幫病人動動看
可以看到只剩幾絲纖維連著

1. 了解 pattern
2. 善用兩邊比較
3. Cellulitis
4. Abcess
5. Fracture
6. Foreign body
7. Tendon tear
